The Sponsor of this Sweepstakes is PROVENIQ Foundation, located at 421 Dougher Knob Rd., Smoot, WV 24977-7060 (the "Sponsor").

The Mountain State $100,000 Challenge (the "Sweepstakes") is open only to legal residents of the state of West Virginia who are at least eighteen (18) years of age at the time of entry. Employees, officers, and directors of Sponsor, and their immediate family members (spouse, parents, siblings, and children) and household members, are not eligible to enter.

The Sweepstakes begins on February 16, 2026 at 12:00:01 AM Eastern Time ("ET") and ends when 100,000 eligible entries have been received OR on August 31, 2026, at 11:59:59 PM ET, whichever occurs first (the "Entry Period"). Sponsor's server is the official time-keeping device for this Sweepstakes.

Limit: One (1) Entry per Household Address.

For the purposes of this Sweepstakes, a "household" is defined as a unique physical street address in West Virginia. Multiple entries from the same address, even if submitted by different individuals or for different pets, will result in the disqualification of all subsequent entries.

There are two (2) ways to enter:

A. App Registration Method (Mayday / LifeLog):

• Download or access the Mayday app (Pre-Registration) or LifeLog application.
• Create a user account and successfully register at least one (1) pet.
• Required Data: Registration must include either the pet's valid 15-digit ISO Microchip ID OR a biometric noseprint record, plus pet name, species, and the owner's valid West Virginia residential address.

Upon successful completion of these steps during the Entry Period, the household will receive one (1) entry into the Sweepstakes.

B. Alternative Method of Entry (AMOE) - Mail-In:

To enter without using the apps, hand-print your full name, West Virginia mailing address (no P.O. Boxes), daytime phone number, email address, and either your pet's 15-digit ISO Microchip ID number OR a linked biometric noseprint reference on a 3" x 5" card or piece of paper and mail it in a business-size (#10) envelope to:

Mail-in entries must be postmarked by August 31, 2026 and received by September 5, 2026. Limit one (1) mail-in entry per household.

On or about September 5, 2026, Sponsor will select one (1) potential Grand Finalist in a random drawing from all eligible entries received. The odds of being selected as the Grand Finalist depend on the total number of eligible entries received, up to a maximum of 100,000 (odds: 1 in 100,000).

Notification: The potential Grand Finalist will be notified by email and/or telephone within twenty-four (24) hours of the drawing. The potential finalist must respond within 72 hours of notification. Failure to respond within this time frame, or failure to provide proof of West Virginia residency and the validity of the registered pet's microchip ID or biometric noseprint record, will result in disqualification and the selection of an alternate Grand Finalist.

The confirmed Grand Finalist will be invited to participate in the "Score Prediction Event" to win the Grand Prize.

• The Event: West Virginia University (WVU) vs. University of Virginia (UVA) Football Game.
• Date: September 19, 2026.
• Requirement: The Grand Finalist must submit a written prediction of the exact final score for both teams (for example, WVU 24, UVA 21) to Sponsor no later than September 18, 2026, at 5:00 PM ET. The prediction must be submitted via the secure digital form provided by Sponsor or via notarized affidavit.

A. Grand Prize (Conditional on Correct Prediction):

If the Grand Finalist's score prediction exactly matches the official final score of the designated game (including any overtime periods):

• Winner Prize: $100,000.00 USD (awarded as a check).
• Charity Prize: $100,000.00 USD donation made by Sponsor to a 501(c)(3) animal shelter or rescue organization located in West Virginia, selected by the Winner (subject to Sponsor verification of non-profit status).

Total Approximate Retail Value ("ARV") of Grand Prize: $200,000.00.

B. Consolation Prize (Guaranteed):

If the Grand Finalist's score prediction is incorrect, they will receive:

• One (1) $500.00 Chewy.com Gift Card for the Grand Finalist.
• One (1) $500.00 Chewy.com Gift Card for the Grand Finalist's selected West Virginia 501(c)(3) animal charity.

ARV of Consolation Prize: $1,000.00.

All entries and the Grand Finalist are subject to verification. The Grand Finalist will be required to sign and return an Affidavit of Eligibility, Liability Release, and (where legal) a Publicity Release, as well as an IRS Form W-9. The microchip ID or biometric noseprint provided at entry must correspond to a real, living animal residing at the entrant's address. Sponsor reserves the right to physically verify the pet's existence if necessary.

General Conditions: Sponsor reserves the right to cancel, suspend, and/or modify the Sweepstakes if any fraud, technical failures, or any other factor beyond Sponsor's reasonable control impairs the integrity or proper functioning of the Sweepstakes. Sponsor reserves the right to disqualify any individual it finds to be tampering with the entry process or acting in violation of these Official Rules.

Limitation of Liability: By entering, entrants agree to release and hold harmless Sponsor, its subsidiaries, affiliates, suppliers, distributors, advertising/promotion agencies, and each of their respective parent companies and each such company's officers, directors, employees, and agents (collectively, the "Released Parties") from and against any claim or cause of action arising out of participation in the Sweepstakes or receipt or use or misuse of any prize.

Disputes: Except where prohibited, entrant agrees that any and all disputes, claims, and causes of action arising out of, or connected with, the Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in Greenbrier County, West Virginia. All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules, entrant's rights and obligations, or the rights and obligations of the Sponsor in connection with the Sweepstakes, shall be governed by, and construed in accordance with, the laws of the State of West Virginia.

Privacy: Information collected from entrants is subject to Sponsor's Privacy Policy. By entering, you agree that your information may be used by Sponsor to administer the Sweepstakes and for marketing purposes in accordance with the Privacy Policy.
